The documents below are from direct communications with DAN & USGC and material from NOAA. There are two very important "forms" attached below that are of particular use for all divers and a third that is helpful in using the first two. The first document is a PowerPoint PPT presentation given to local divers in Michigan. It is an EXCELLENT place to start and will introduce how the documents that follow can/should be used. It is filled with a lot of practical information on "how to manage" a post accident dive event. The samples of the documents in this presentation are taken from the document below. The "forms" are designed to be complete, yet simple and QUICK to employ. During a post dive accident you need to "capture" much important & relevant information to pass to the EMS & hyperbaric medical staff. You don't have time to write a "novel" … so the forms assist you in this vital documentation process.

- First is the URA Diver Emergency Transportation document. This document provides instruction on how to provide transport for a diver suspected of a major medical emergency AND MAY have diver associated issues as well (e.g. DCS, ACE-AGE, etc.) Here you will find specific contact numbers and information on how to prep for transport and how to follow up on transport. The instructions are 100% compliant with DAN's recommendations. This form helps you start the procedures to transport a diver to a major EMS facility & then subsequently connect the EMS to DAN and determine hyperbaric treatment options. - Second is the URA Diver Emergency Incident Form which assists in capturing all of the necessary information for the EMS and (if necessary) the hyperbaric facility treatment. This document is designed to be sent along with the diver in transport for both the EMS facility & the hyperbaric facility. It provides the diver's name & contact info, the symptoms and vital signs taken on site prior to transport, the dive profile, and the on-site neurological exam data and more. Remember to take your cell phone and take an image of all pages of this document PRIOR to sending it along in transport. (Things get lost & this data can provide VITAL INFORMATION to the EMS & hyperbaric facilities to proper treat the diver. Filling out this form has an extremely high priority. Assign one person in your group to enter and document everything done on the form. Current contact info is included.

- Most divers do not understand why you should call 911 first and DAN second. The following document from DAN explains why this is necessary. It also explains why DAN does not give out hyperbaric facilities (with two exceptions) to normal divers. Included are explanations on why an EMS facility is the primary first place to send a diver suspected of any major illness ... including DCS/ACE-AGE, etc.

- The DAN document below (DAN Dive-Travel Guide) is a really extensive document that covers all types of first aid (marine/coral animals & much more). There is a nice summary of diver illness symptoms as well as normal first aid issues and priority. Included are some travel tips, health suggestions, etc. A really nice document that covers a wealth of issues that divers may have need of understanding.

- Finally, the last document is a collection of many resources I found at DAN's website, and in communications with DAN's medical information call line, as well as several direct communications I had with the USCG. Much of this information was utilized in the drafting of the two top URA reporting forms. While more of a "useful reference" the first two documents above are the "meat & potatoes" of what every diver should understand about suspected diving accidents.
